Medication and Medical History
Make sure to have a list of any prescriptions or herbal medicines you’re currently taking and be sure to tell us if you have specific health problems we should know about over the phone.
Dentist and Physician
Bring the names and contact numbers of your primary physician, your dentist and any medical professionals you’re currently seeing.
Films & Referrals
If you have x-rays and/or referral forms from your physician or dentist, we’ll certainly need to see them. You may contact your dentist or physician to forward these to our office at your request.
Insurance Info
Be prepared to supply us with both your medical and dental insurance information, and bring your insurance cards and photo identification.
Please Note: All patients under the age of 18 must have a parent or guardian present for both consultation and surgery appointments.
