A Policy Designed for Your Convenience
Lafayette’s Oral Surgery Office (OSO) will work with you toward the payment of your surgical procedure, and our staff is equipped to make the process as easy as possible.
Filing a Claim
OSO will gladly file insurance claims on your behalf. Please be sure you provide all of your medical and dental insurance information ahead of your first visit. You can provide it over the phone when you make your appointment, or you can present it when you arrive. This helps us determine the amount of your co-pay that will be paid on the day of your surgery. It is the patients responsibility to call and speak with your insurance company to verify benefits or determine if you need a prior authorization for our services. We will do our best to help, but you will ultimately be responsible for all charges your insurance company does not cover for any reason.
We are providers for Delta Dental, and multiple Anthem Dental plans.
What else should I bring to my appointment?
Making a Payment
If you do not have insurance, that is not a problem. We can give you an estimate of the charges for the services you plan to have at our office. This would be due the day of your surgery. We accept Mastercard, Visa, Discover and Care Credit.
